To be fair to Andrew, I did hear from him with apologies for the delay, soon after I posted his letter on Updates. He also sent me the link for the questionnaire which I duly completed. Perhaps any more queries on this could be addressed directly to Andrew at the email address Joe provided for him. I must say I am surprised, with all the disquiet and conjecture about this survey, that I seemed to be the only person who thought to pose the questions to the guy who set it. For what it's worth I wasn't impressed by the survey mostly because of the problem I have with all questionnaires of this kind which pose rigid questions and then ask you to tick boxes with agree/disagree etc. The questions are never specific enough and because the words used may have varying meanings or interpretations, one feels it is impossible to give a meaningful answer.
